Roudebush VA Medical Center (VAMC). \nAll work will be performed at the Richard L. Roudebush VA Medical Center (VAMC), located at 1481 West 10th Street, Indianapolis, IN 46202. The Contractor shall complete all work within 90 days following Notice to Proceed. The concrete repair work shall be completed during normal hours and on the weekend. All parking lots and roadways should be fully operational during business hours. \nThe Contractor shall provide all necessary services (i.e., labor, materials, tools, equipment, and competent supervision) to perform all work as noted below to meet the scope objectives and requirements, and to provide the required outcome in terms of overall quality, timeliness, and documentation. The Contractor shall be responsible for obtaining and coordinating the services of all third-party subcontractors performing services on site as part of the contract to accomplish the following:\nRemove and replace concrete ramps, sidewalk, curbs, and detectable warning strips as shown on the drawings. All pedestrian ramps and sidewalks shall be VA compliant. https://www.cfm.va.gov/til/etc/dsBarrFree.pdf \nPrior to start of work, the Contractor is responsible for verifying existing conditions and maintaining current conditions of the VAMC during and following completion of the installation. \nContractor shall mobilize equipment and materials without interfering with the normal flow of vehicles, pedestrian traffic, and medical center operations.\nIt shall be the responsibility of the Contractor to keep the construction areas clean and the surrounding areas free from damage. If during work, surrounding areas are damaged, the Contractor will be responsible for any repairs/replacements that are required.\nIt is the responsibility of the Contractor to understand and comply with all applicable city, county, state, and federal codes, laws, ordinances, standards, rules, and regulations. \nThe contractor is responsible for materials storage until the materials are installed. The VA will not allow storage in the medical facility except for inside the construction area. \nThe contractor shall notify the COR daily when their crew is on site working. \nWORK HOURS\nMaintaining the operational capability of the medical center is of paramount importance. The contractor shall coordinate all activities with VA COR. The concrete work can be completed during normal business hours, but weekend work will be necessary for certain areas. All after-hours work or weekend work shall be approved by the COR. Normal business hours for the medical center are defined as Monday through Friday, 0730 1630, excluding Saturday and Sunday. Designated off-shift hours are defined as 1630 0700, Monday through Friday, excluding federal holidays. \n\nThe following construction activities shall be considered after-hours work.\nWork that is loud or disruptive that causes impacts to patients or patient care. \nWork that impacts employee and patient parking or traffic flow. \n\nHEALTH & SAFETY \nThe Contractor shall be thoroughly familiar will all rules and regulations governing safety. A 30-hour OSHA Competent Person shall be present for all work and all additional workers shall have a minimum of a 10-hour OSHA Certification. All welders, pipefitters, carpenters, electricians, etc. performing work must present a valid certification for their respectable trade. The contractor shall ensure all OSHA requirements are followed specifically related to controlling silica dust. The contractor shall implement controls to eliminate dust. \nAll work associated with this project shall be in compliance with all VA regulations concerning Safety, Infection Control Requirements, Interim Life Safety Measure Requirements, Parking, Personnel Access, and proper removal/safe disposal of construction waste and debris as listed in the specifications and drawings. The Contractor shall abide by all Infection Control Risk Assessment (ICRA) requirements that are put into place by the VA infections prevention team. The COR shall provide requirements and forms to Contractor. \nAll work will be performed in accordance with ICRA, but it will still be the responsibility of the Contractor to keep the construction areas clean and to not damage the existing equipment. If during the work, equipment is damaged, the Contractor will be responsible for any repairs/replacements that are required. \nStarting October 1, 2019, the VAMC campus has become a Smoke-free zone. No smoking is allowed on the grounds of any VA-related health care facilities by patients, visitors, volunteers, contractors, vendors and employees. The new smoke-free policy applies to cigarettes, cigars, pipes, any other combustion of tobacco, and e-cigarettes, vape pens and e-cigars. \nA safety plan shall be approved by the VA before any work is started. Once the safety plan is submitted, a meeting will be setup with VA safety, the COR, and the Contractor to review and discuss the safety plan.\nChemicals shall be properly stored and approved by the VA. \nThe contractor shall provide all barricades, signage, and fencing required to protect and secure the construction site. The concrete related work shall require fencing, and signage during demolition and installation. The asphalt related work shall require signage, barricades or danger tape to keep people from entering the work area. \n\nREQUIREMENTS \nContractor shall conduct a weekly progress meeting with the COR. The Contractor shall provide the COR with an updated two-week schedule, review submittals and RFIs with the COR, provide documentation for any outages or closures and discuss project progress. \nThe Contractor shall provide the COR with a daily log of any work that was included that day. The daily log shall include who was on site, their trade, how many hours were worked and what work they were performing. \nThe contractor will be authorized for 1 vehicle on-site. Additional space if needed will need to be at the Cold Spring Roads facility. \nA dumpster may be authorized onsite if required. A dumpster will be authorized if there is space on site available and if approved by the COR. \nThe Contractor shall be responsible for all signage Wayfinding signage shall clearly indicate areas that are closed off to the public.
