Statement of need \nVAPORHCS Pathology and Laboratory Medicine Service require a supplier to deliver dry ice for use by the send out department for shipping and packaging referral laboratory specimens. P&LMS is seeking a contract duration of 5 total years, 1 base year plus 4 option years. \n2. Description of supplies needed \na. 100lbs of pellet or nugget dry ice delivered weekly. Dry ice must be scoopable. Facility has a 100lb container that the vendor will place the dry ice in. \nDesired size of dry Ice pellet or nuggets ½ to 5/8 IN diameter and 1.5 IN in length.\n3. Delivery information \nDeliveries will be made on the first day of the week after the weekend which will usually be a Monday. In the case when Federal holidays fall on Monday (see link below) the delivery will be made on Tuesday. \nDelivery to take place on the VA Portland campus:\nPortland VA Hospital\n3710 SW US Veterans Hospital Road\nPortland, OR 97239\nBuilding 100, Floor 2; 2C-107\n\n*Follow instructions on the wall to gain entrance.
