The priority is to find two or more Service-Disabled Veteran Owned Small Businesses (SDVOSB), Veteran-Owned Small Businesses (VOSB).\n\nDocumentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition.\n\nResponses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number and (h) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below.\n\nCONTRACT REQUIREMENTS\nService Contract to have a Preventative Maintenance and Repair Service for the Bathrooms, Kitchen and wet areas at the Phoenix Veterans Hospital. \n\n\nDESCRIPTION / WORK STATEMENT:\n\nThe contractor shall provide all qualified personnel (labor), material, equipment, technical expertise, tools, travel expenses, supervision, transportation, and necessary parts to provide an annual comprehensive preventative maintenance and repair service for bathrooms, Kitchen and wet areas for the Environmental Management Services (EMS) Department at Phoenix VA Health Care System (PVAHCS) in Phoenix, AZ. \n\nThe contractor shall provide a supervisor for the full duration of the project and he/she must be on site at all times while work is being performed. Contractor shall provide Quality Assurance (QA) person and shall inspect, test and perform duties consistent with contractor submitted quality assurance program. Contractor s QA shall provide daily reports to the government project manager on a weekly basis: this report shall show in detail the subcontractor s work performance on that day. Report shall include any findings by the QA on that day. The contractors QA and superintendent cannot be the same person, unless the CO has specified otherwise. Contractor shall submit all products used, to include Safety Data Sheets (SDS) to the CO before and all items are brought or installed on projects.\n\nThis project will consist of the following elements but is not limited to these: pre-restoration work necessary before commencement of restoration process steps. Examples include removal of floor coatings/ caulking/polyurethane removal and replacement of partitions or work area impediments, grout sawing in cases where grout depth is insufficient to achieve permanent bonding of glazing compound. Below are some features of work but these are not all inclusive of all work needed to be accomplished on this project. It is the contractor s responsibility to define and capture all work required to give the government a complete correction of the condition and appearance of the Medical Center floors. Contractor will clean, sanitize and prep the floors, followed by the SaniGlaze restoration process that will protect the tile and grout from contamination.\n\nFloor work located throughout building 1, 8, 16, 21, 23, 34 and ACC. Contractor will utilize solutions and equipment to emulsify and extract contaminants and odors from the tile and grout. Contractor will apply a nonporous polymer coating on all grout that will permanently bond to existing grout lines. Incases where grout depth is insufficient to achieve permanent bonding of glazing compound, grout sawing may be required. Contractor will prepare the entire surface to apply a chemical resistant coating that will shield the floor from future contaminates.
