{"description":"SCOPE OF WORK \n\n1.0 Introduction\n\nThis Statement of Work (SOW) defines the requirements of the Harry S Truman VA Medical Center - Multi-Specialty CBOC Rolla in establishing a fixed-price brand name or equal order for dental lab tools.\n\n1.1 Background\n\nHarry S Truman VA Medical Center - Multi-Specialty CBOC Rolla requires dental lab tools to outfit the new facility and provide services for the VA.\n\n1.2 Scope\n\nThe scope of this purchase order is to provide Harry S Truman VA Medical Center - Multi-Specialty CBOC Rolla with brand name or equal item for delivery to the Harry S Truman VA Medical Center - Multi-Specialty CBOC Rolla location. Contractors shall furnish all requirements. The initial delivery will be to an offsite warehouse to support installation timelines. The warehouse location is yet to be determined. The warehouse will be available June 1st, 2026 May 31st,2027. Initial Outfitting Transition and Activation team will be responsible for delivery to Rolla Super CBOC estimated delivery dates are November 2nd, 2026 January 8th, 2027. Specific installation dates will be defined upon award.\n\nThe delivery schedule is based upon the current construction schedule, which is subject to change. The vendor shall not increase prices for a change in delivery date due to construction delays. The awardees actual delivery dates will be confirmed by VA upon award.\n\n2.0 General Requirement \n\nAll equipment to be supplied under the specifications shall be the newest and the most current model of a standard product of a manufacturer of record. A manufacturer of record is defined as a company whose main occupation is the manufacturer for sale of the items of equipment and which:\n\na. Maintains a factory production line.\nb. Maintains a stock of replacement parts for the item.\nc. Maintains engineering drawings, specifications, operating manuals and maintenance manuals.\nd. Any adjustments to pricing, schedule, or resources resulting from such changes shall be addressed through the appropriate contractual modification process in accordance with the applicable Changes clause.\n\n3.0 Work Hours:\n\nNormal business hours are 8:00AM to 4:30PM Monday thru Friday excluding Federal Holidays. Work completed outside this time shall be requested through the Point of Contact.\n\nThe Government recognized US holidays are: New Year's Day, Martin Luther King, Jr. Day, Presidents Day, Juneteenth, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. If the holiday falls on Sunday, it is observed on Monday.\n\n4.0 Delivery and Receiving:\n\nDelivery and receipt of the proposed items is anticipated to be directed to the location identified below. Confirmation of delivery location will be provided by the VA POC post-award. Any item that has not been coordinated will be turned away and any additional charges will be the responsibility of the sender/transportation company.\n\nDelivery shall be freight on board (FOB) Destination.\nWarehouse\n2701 Vandiver Dr. Columbia, MO 65202\n\nInstallation Address:\nHarry S Truman VA Medical Center - Multi-Specialty CBOC Rolla \n13063 Old Wire Rd\nRolla, MO 65401-000\n\nVendor Delivery paperwork requirements:\n Vendors must provide packing slips with a line-by-line inventory for each item in the delivery to include description and serial number for each item so that items can be entered into the VA property book upon arrival to site or warehouse.\n\n5.0 Period of Performance: \n \nPeriod of performance will be from November 2nd, 2026 January 31st, 2027 giving the vendor time to deliver the product and install, to each listed location, after award. \n \n6.0 Installation \n \nThe contractor shall be responsible for the installation, performance verification and validation of the structural components with a VA Activations member who will accept the equipment. \n \nAll requested products shall be completely manufactured as specified and delivered to the respective delivery locations as described above. \n\nWhite glove installation shall be included for all products provided under this award.\n\nCoordination will need to occur between COR, Activations PM and the GC for utility connections. \n\n7.0 Warranty and Support:\n\nThe contractor shall provide a manufacturer s two-year, free replacement warranty on units with normal wear and tear, including free return shipping, from date of delivery.\n\n8.0 Use of Premises\n\nIf during the performance of this contract, work will be carried out on Harry S Truman VA Medical Center - Multi-Specialty CBOC Rolla premises, the vendor shall perform all work in such a manner that will cause a minimum interference with operations and the operations of other contractors on the premises.\n\nThe vendor shall communicate through the VA POC any required coordination requirements with the General Construction contractor prior to and during delivery and installation activities.\n\nThe project may be an active construction site during delivery and installation. The vendor shall take all necessary precautions to protect the premises and all persons and property therein from damage or injury. The vendor shall assume responsibility for taking precautions for the vendor's (and associated subcontractors ) employees, agents, licensees, and permittees. The vendor shall abide by any safety requirements imposed by the GC (e.g. hard hats, safety vests).\n\n9.0 GOVERNMENT AND CONTRACTOR RESPONSIBILITIES\nVA shall provide the contractor with copies of documents that the VA is required to provide.\nThe contractor shall request other VA documentation deemed pertinent to the work accomplishment directly from the COR. The contractor is expected to use common knowledge and resourcefulness in securing all other reference materials, standard industry publications, and related materials that are pertinent to the work.\nThe contractor shall maintain frequent communications with the Program Office and the COR to conduct work in progress reviews if necessary.
