In-person assessments: assessors must be located in the relevant geographical region to deliver face-to-face services as well as have the ability to realistically manage their capacity across multiple suppliers. To deliver the services, Suppliers will need to have at least one Named Provider approved on their contract. Each Named Provider must meet the eligibility criteria specified in the Service Schedule. Submitting an Application: - All new Respondents are required to answer ALL questions in the Application Response Form. In addition to this, they are required to complete an individual OAS Named Provider form for each Named Provider and an OAS Named Provider Service Coverage spreadsheet. - Respondents who were unsuccessful in the previous tender must complete the relevant sections in Section 5 (Service Requirements) of the Application Response Form that either failed or received a conditional pass, unless there have been any changes to the sections that previously passed.
