This professional service aids in preventing the buildup of specs, particles, flakes and debris on surgical trays used for sterile medical procedures.\n\nFor several months the SPS department found foreign objects, specs and debris in surgical instrument trays and in operating handpieces. In order to maintain a sterile environment, the washer-sterilizers need to be professionally cleaned regularly.\n\nREQUIRED SERVICES \nMonthly Cleaning of Washer-Sterilizer #1 and #2 located in Building 1, Room 1426.\nThe Contractor shall provide all parts, travel, and labor.\nThe Contractor shall coordinate all services with the Contracting Officer s Representative. \nThe Contractor shall provide one-week notice prior to coming on site to perform services. \nThe Contractor shall check-in and check-out with the Sterile Processing Service located in Building 1, Room 1403.\nThe Contractor shall verify all equipment was serviced is functional before leaving the medical facility.\nThe Contractor shall deliver a hard copy or electronic copy of a Field Service Report that delineates the service performed and results to COR.\nThe Contract shall provide for the addition and/or removal of equipment and/or services.\n\nDELIVERABLES\nFiled Service Reports (FSR) shall be provided for all service activities and shall contain the following information:\nDate(s) and time period of service.\nComplete description of equipment serviced including model number and serial number.\nComplete description of equipment services performed. \nThe POC is Patrick Edem at Patrick.edem@va.gov \n\nPERFORMANCE MONITORING \nThe Agency shall oversee work done by the vendor and accept the quality of work as appropriate. \n\n\nOTHER PERTINENT INFORMATION OR SPECIAL CONSIDERATIONS\nInspection and Acceptance Criteria: The Agency shall sign off on the work completed if service is determined to be satisfactory.\nThe Agency shall provide utilities necessary for Contractor equipment used under this contract. \nThe Agency shall provide physical access to the equipment.\nThe Agency and contractor will coordinate date and time when contract is awarded.\n\nSecurity: \nIn reference to VHA Handbook 6500.6 Appendix A, Block 6, the service included in this contract does not involve connection of IT devices to a VA network.
