Tender summary
The goal of this project is to improve the early identification, clinical management, and long-term health outcomes of individuals with hemoglobinopathies in Mississippi by ensuring timely newborn screening follow-up, coordinated medical case management, patient and family education, and strengthened linkage to specialty care and community resources. The project also aims to increase awareness and interdisciplinary team knowledge of hemoglobinopathies through outreach, education, and collaboration with the Mississippi State Department of Health Genetics Program. The Contractor will provide medical case management for patients with hemoglobinopathies referred by the MSDH Genetics Program, including newborn screening follow-up, family testing when needed, and education or counseling to support early treatment and intervention. The contractor will also collect and report aggregate patient quarterly, participate in community outreach and education activities in Mississippi, provide clinically accurate educational materials and training opportunities for relevant audiences, share newborn screening referral results with MSDH district genetic coordinators, and designate a qualified representative to participate in MSDH Genetics Program virtual and regional trainings or advisory groups meetings, as needed.
