Secure Document ShreddingServices- Secure document shredding services provided by a NAID (National Association forInformation Destruction) AAA certified vendor who can provide confidentialdestruction of paper records, microfilm, microfiche and other media as needed. Theservice provider shall pick up and remove confidential material for off-sitedestruction or provide on-site destruction services. The service provider shallprovide all materials, labor, vehicles, equipment, supplies and incidentalsnecessary to perform the services not specified as TSLAC responsibilities, toprevent unauthorized access, identity theft, and data breaches. These services mustensure compliance with applicable privacy, records management, and informationsecurity requirements. The Statement of Work outlines the requirements for thesecure destruction of confidential and sensitive documents to ensure compliancewith applicable privacy, security, and data protection regulations. Mostservices will be for paper records in one cubic foot standard-size boxes; paperdocuments must meet the ISO/IEC21964 level P-4 shred standard of strips nolarger than 6mm (.24 in) X 25mm (.98in ), which are palletized. Other services mayinclude secure bin collection of paper that could include set schedules forpick up of full containers and drop off of empty containers. The serviceprovider shall perform all work in a manner that guarantees completedestruction beyond recognition or reconstruction. Work may be completed on-siteor off-site at a service provider’s location. Work may be completed on-siteor off-site at a service provider’s location. The contractor shall: Collect materials on a scheduled or on-demand basis. Shred documents using cross-cut or micro-cut methods meeting or exceeding industry standards (e.g., DIN 66399 P-4 or higher). Paper documents must meet the ISO/IEC 21964 level P-4 shred standard of strips no larger than 6mm (.24 in) x 25mm (.98 in). Ensure destruction of paper, microforms, optical media, and other physical storage devices as required. Provide secure collection containers (locked, tamper-resistant) for designated locations for non-boxed confidential media. Provide at minimum off-site shredding, with chain-of-custody documentation. SecurityRequirements All personnel handling materials must pass fingerprint-based criminal history checks and sign a confidentiality agreement. Materials must remain under continuous supervision until destruction is complete. Vehicles used for transport must be locked at a minimum. (GPS tracked would be ideal) The contractor must comply with all applicable federal,state, and local regulations, including but not limited to: IRS Publication 1075 HIPPA Privacy Rules (45 CFR 164.530 (c)) Internal Revenue Manual 1.15.3 Texas Government Code Chapter 441, Subchapter L Texas Local Government Code Section 202.003 Texas Administrative Code, Title 13, Chapter 6 Texas Administrative Code, Title 13, Chapter 7 Certification of Destruction Upon completion of each shredding service, the contractorshall provide a Certificate of Destruction indicating: Date and time of destruction Method used Location of destruction Authorized signature Ifthis can’t be provided, auditable documentation must be provided.
